STAFF
We wish your experience with Crockett Prosthetics & Orthotics to be professional
and courteous, while meeting all of your functional needs.
If, at any time, you feel that we might improve our efforts,
we welcome your comments and suggestions graciously. We are committed
to service, innovation and excellence.
HOSPITALS
If you find yourself in a local hospital or health-care facility
and need new service, adjustments or repairs to an existing
appliance, give us a call, we will see you in-house. Generally,
your physician needs to write an order or request a consultation
with our facility to begin this process.
WARRANTY & ADJUSTMENTS
Follow-up visits are a normal and routine course in prosthetic
and orthotic services. We may recommend a return appointment
at time of delivery to monitor your progress or make adjustments
we anticipate that will be necessary. If repairs are necessary,
it is the patient's responsibility to schedule appointments
and convey the nature of the problem as quickly and concisely
as possible. At no time should you attempt to adjust or
repair your appliance yourself. This may void any warranties
and cause you to incur unnecessary expenses. We will do everything
possible to repair your appliance as quickly as possible to minimize
your inconvenience.
INSURANCE
As a courtesy to our patients, we will normally file your
insurance after preparing a claim form for the services
we provide to you. To facilitate this procedure, you must assign benefits
to us. Any balance not covered by your insurance is your responsibility
and payable upon delivery unless prior arrangements are
made with our billing department.
