 Peter Paul Kosloski, CPO, LPO, FAAOP

Licensed and Certified Prosthetist/Orthotist
Paul is a graduate of The University of Memphis where he obtained a Bachelor of Science degree in Microbiology and Botany. He obtained his Orthotic education from Shelby State and his Prosthetic education from Northwestern University. His internship was completed at Oklahoma University. Mr. Kosloski is also a Fellow of the American Academy of Orthotists & Prosthetists, which is a very distinguished accomplishment and shows his commitment to always continuing his education in the field as well as a commitment to excellence.

Licensed and Certified Prosthetist/Orthotist
Leslie has been with Crockett since 2000.
Leslie is a graduate of The University of Tennessee in 1995 where she earned a Bachelor of Science degree in Exercise Physiology and Kinesiology. She works with all types of patients. As a Prosthetist, Leslie evaluates and fits each patient for the appropriate type of prosthesis. As an Orthotist, Leslie evaluates for and fabricates any type of orthosis a patient requires including arch supports, knee orthoses, and ankle-foot orthoses. Leslie oversees all patient management at all of Crockett facilities.

Kelly has been with Crockett since August 2002.
As a Facility Administrator, Kelly handles the majority of our company’s insurance claims, contracts, business accounts, patient relations, and special projects. Kelly is also Board Certified and carries her own patient load. She sees patients for items such as diabetic shoes, mastectomy products, knee orthoses, lumbo-sacral orthoses, and compression garments.

 Brandon M. Jones C.Ped., CFo
Board certified Pedorthist and Orthotic Fitter.
Brandon has been with Crockett since Feb. 2008. As a Pedorthist, Brandon works with a physicians prescription to design, fabricate and dispense custom made therapeutic devices such as foot orthoses, partial foot prostheses, subtalar control foot orthoses, and custom diabetic foot wear. As an orthotic fitter, Brandon fits patients with a wide range of pre-fabricated orthotics for the upper and lower extremities.

 Patricia “Pat” Lane Jones
Prosthetic and Orthotic Technician/Fitter
Pat has been with Crockett since May 2002.
Pat is an integral part of the Crockett team. Pat fabricates almost all of the “in-house” custom work such as custom arch supports, custom ankle-foot orthoses, and prosthetic sockets. She is also in the process of obtaining her Orthotic Fitter and Mastectomy Fitter Certification. She is the second pair of hands for the practitioners with any of the patients, prosthetic or orthotic.

 Arisa Pomhin
Office Assistant
Arisa’s responsibilities include insurance authorizations, patient communication, scheduling, and orders. Arisa is our newest employee and has a bright future ahead of her. Arisa is a great addition to our company and we are looking forward to seeing her grow and come into her own in this field.
